The objectives of a well-run M&M conference are to learn from complications and errors, to modify behavior and judgment based on previous experiences, and to prevent repetition of errors leading to complications. These meetings highlight recent cases and identify areas of improvement for clinicians involved in the case. They are also important for identifying systems issues which affect patient care. With over 25 years of experience as a “doctor” of identity management Jackson Shaw will present his mortality and morbidity findings for identity management. He will highlight some recent patients that have died in the operating room, others who have been left worse off than before they underwent “surgery” and some “patients” who simply have no hope.
